Indications
Clomipramine is prescribed for the treatment of Obsessive-Compulsive Disorder (OCD). Its therapeutic effects are attributed to the reduction of unwanted, persistent thoughts (obsessions) and the mitigation of the compulsion to engage in repetitive behaviors (such as hand-washing, counting, or checking) that impede daily functioning. Clomipramine belongs to a class of medications known as tricyclic antidepressants, which exert their effect by restoring the equilibrium of certain endogenous neurotransmitters, including serotonin, within the brain.
Uses and Dosage
Clomipramine is administered orally in the form of capsules. Typically, during the initial stages of treatment, the medication is taken three times daily with meals to allow for proper adjustment of the body to the drug. After a few weeks of therapy, the medication is typically taken once daily at bedtime.
Adhere to the instructions on the prescription label and seek clarification from the physician or pharmacist on any unclear directions. Clomipramine should be taken exactly as prescribed, and the dosage should not be increased, decreased, or taken more frequently than directed by the physician.

Caution
- Prior to taking clomipramine, inform your healthcare provider of any allergies you may have, particularly if you have had an allergic reaction to other tricyclic antidepressants such as imipramine or nortriptyline, or if you have any other allergies. - Prior to using this medication, be sure to disclose your medical history to your healthcare provider or pharmacist, particularly if you have a history of bleeding disorders, blood disorders such as purpura or thrombocytopenia, respiratory issues such as asthma or chronic bronchitis, a personal or family history of glaucoma of the angle-closure type, eating disorders such as bulimia, heart problems such as arrhythmias, coronary artery disease or recent heart attack, intestinal issues such as chronic constipation or ileus, liver or kidney problems, personal or family history of other mental/mood conditions, heartburn or stomach acid in the esophagus such as due to hiatal hernia, seizures, overactive thyroid (hyperthyroidism), difficulty urinating, any condition that may increase the risk of seizures such as alcohol or sedative dependency, use of electroconvulsive therapy, brain injury/disease or certain types of tumors such as pheochromocytoma or neuroblastoma.
- Clomipramine may cause QT prolongation, a condition that affects the heart rhythm. QT prolongation can rarely result in serious (and rarely fatal) fast/irregular heartbeat and other symptoms such as severe dizziness or fainting which require immediate medical attention.
- This medication may cause dizziness or drowsiness or temporarily blur your vision. Alcohol or marijuana can increase these side effects. Do not drive, operate machinery, or engage in any activity that requires alertness or clear vision until you can do so safely.
- Older adults may be more susceptible to the side effects of this medication, particularly bleeding, confusion, dizziness, and QT prolongation. Dizziness and confusion can increase the risk of falling. Older adults may also be at greater risk of developing low sodium in their blood, particularly if they are taking diuretics.
- During pregnancy, this medication should only be used when it is absolutely necessary. Newborns exposed to clomipramine during pregnancy may experience withdrawal symptoms or side effects. Discuss the benefits and risks with your doctor, and inform them immediately if you notice jitteriness, shaking, feeding problems, fast breathing, or seizures in your newborn.
- Since untreated mental/mood problems such as obsessive-compulsive disorder, depression, or panic attacks can be serious conditions, do not stop taking this medication unless directed by your doctor. If you are planning to become pregnant, are already pregnant, or think you may be pregnant, discuss the benefits and risks of using this medication during pregnancy with your doctor.
- This medication passes into breast milk and may have adverse effects on a nursing infant. Speak to your doctor before breastfeeding.
